Danielle first discovered Chronic Care Management in 2017 when joining Rhythm Management Group as their CCM Director. During that time, CCM wasn’t well known or utilized with less than 5% of providers offering CCM services. After managing cardiac Device remote monitoring for several clinics, Danielle was approached again by administration to create and manage a new CCM service line. This led to Danielle building a comprehensive and successful CCM and RPM Platform for the company that launched on January of 2020, just before the Covid health pandemic. The platform she created is still in use at RMG, and currently monitors over 20,000 patients nationally with a 94% patient satisfaction rate.
